This seems esp important when vaccinating young children or fragile patients. I would not recommend a thiomersal-based vaccine for a patient who already shows signs of a mercury load. Unfortunately, there is no ideal diagnosis, although we have made progress. 1. blood analysis can be utilized simply because ranges have been lowered and instrumentation improved to the point where we see miniscule amounts. Preferably, test results are negative, but that is rarely the case. If we see Hg, action is needed. 2. hair mineral analysis. As long as the body is able to excrete, we see Hg in hair. Any amount seen asks for action. I am quite aware that present reference ranges need to be updated, and that does apply to all laboratories I know. Instrumentation and techniques have improved and analytically, we can utilize hair analysis to patients’ advantage. 3. Talking with patients about potential exposure is important. Many young mothers are unaware that they have detoxified themselves through pregnancy, unknowingly burdening their young- particularly when dental work was done prior to and during pregnancy. There are other potential exposures we often forget about such as eating large fish (tuna, swordfish) too often, or using Hg-containing disinfectants excessively etc etc. Accumulation is a problem and we cannot ignore a little bit here and a little bit there. It all adds up and accumulates, burdening systems, esp. the young and fragile 4. When we see mercury in any small amount in blood or hair, consider a challenge. The ideal chelators for a mercury detox would be DMPS, DMSA, even S-containing amino acids can be used to detox depending on the patient’s age and status (see Dr. vanderSchaar’s updated Textbook on Clinical Metal Toxicology). 5. Needless to say, prevention is easier than cure, but a healthy and relatively clean system can handle a bit of thiomersal, if need be (and of course I question the ‘need’) 6. Do I get flu vaccination for myself? NO! Do I consider it for my 1 ½ year old grandchild who came into this world with a small mercury burden, the answer is No. Do I consider any vaccination for her?
